NOTES:
APPLICATION FOR MEMBERSHIP IN THE ITO
To become a member of the ITO, the completed membership application must be sent to the ITO HEAD OFFICE. For minor applicants, we also need the written consent of the parents. On the transfer of single admission fee is necessary to give as USE the word "membership" and the NAME OF THE MEMBER.
After receipt of all required documents, fees and contributions, the ITO of admission will soon be sent to the new member.
ORDER ITO PASS
A pass-order can only be recognized by the ITO on a coach. be the association, the school or institution of the purchaser, must be a member of the ITO. For individual membership, the order can be done by the customer. Bulk orders are possible. ORDER ANNUAL STAMPS
Years marks may only be ordered through an authorized trainer of the ITO. For single membership, the order can be made by the purchaser. BELT GRADE RECOGNITION
The recognition of Belts that have not been removed by a licensed examination of the ITO and / or ITO-Pass confirmed, can be requested via this form.
APPROVAL FOR DAN TESTING
Prerequisite for admission to the Dan-examination of the ito is a recognized degree belt and recognition of the Dan-examination.
REGISTRATION KUP/KYU EXAMINATION
Kup / Kyu examinations can be carried out by auditors who are in the ITO as an examiner or examiners A B registered. Candidates must have a valid passport ITO. In addition, a minor parental consent is necessary. EXAMINER LICENCE B

The minimum age for examiners B is 21. The examiner's license is valid for 2 years and is renewable for an additional fee for each 2 years.
PARENTAL CONSENT FOR MINERS
On this form the parents or guardians consent to their minor child that may participate in the form specified in the event (course, test, tournament, etc.).
